      其敏感性和特异性分别为95%和85%•需强调指出的是,患者病情的初始或许不具备分类标准中的4条,随着病情的进展方出现其他项目的表现•11条分类标准中,免疫学异常和高滴度抗核抗体更具有诊断意义一旦患者免疫学异常,即使临床诊断不够条件,也应密切随访,以便尽早作出诊断和及时治疗  SLE病情估计病情估计•各种SLE的临床症状,尤其是新近出现的症状,均可能提示疾病活动•与SLE相关的多数实验室指标,也与疾病的活动有关 提示提示SLE活动的主要表现有:活动的主要表现有:•中枢神经系统受累(可表现为癫痫、精神病、器质性脑病、视觉异常、狼疮性头痛、脑血管意外等,但需排除中枢神经系统感染)•肾脏受累 (包括管型尿、血尿、蛋白尿、白细胞尿)•血管炎•关节炎•肌炎•发热•皮肤黏膜表现(新发红斑、脱发、黏膜溃疡)•胸膜炎•心包炎•低钠血症•DNA抗体滴度增高•血象三少 (需除外药物所致的骨髓抑制)•血沉增快 国际上通用的几个SLE活动性判断标准包括:•SLEDAI( Systemic Lupus Erythematosus  Disease Activity Index)•SLAM ( Systemic Lupus Activity Measure ),•BILAG等。

      SLE病情轻重程度的估计病情轻重程度的估计•轻型SLE•中度活动型狼疮•重型SLE•狼疮危象 SLE病情轻重程度的估计病情轻重程度的估计•轻型SLE:诊断明确或高度怀疑者,但临床稳定且无明显内脏损害•中度活动型狼疮是指有明显重要脏器累及且需要治疗 •重型重型SLE::①心脏:冠状动脉血管受累,Libman-Sacks 心内膜炎,心肌炎,心包填塞,恶性高血压;②肺脏:肺动脉高压,肺出血,肺炎,肺梗塞,肺萎缩,肺间质纤维化;③消化系统:肠系膜血管炎,急性胰腺炎;④血液系统:溶血性贫血,粒细胞减少(WBC<1,000/mm3 ),血小板减少(<50,000/mm3 ) ,血栓性血小板减少性紫癜,动静脉血栓形成;⑤肾脏:肾小球肾炎持续不缓解,急进性肾小球肾炎,肾病综合征;⑥神经系统:抽搐,急性意识障碍,昏迷,脑卒中,横贯性脊髓炎,单神经炎/多神经炎,精神性发作,脱髓鞘综合征;⑦其他:包括皮肤血管炎,弥漫性严重的皮损、溃疡、大泡,肌炎,非感染性高热有衰竭表现等 狼疮危象•是指急性的危及生命的重症SLE包括急进性狼疮性肾炎、严重的中枢神经系统损害、严重的溶血性贫血、血小板减少性紫癜、粒细胞缺乏症、严重心脏损害、严重狼疮性肺炎、严重狼疮性肝炎、严重的血管炎等。

      诊断依据   1.具有SLE的临床表现及实验室检查证据 2.有比较明确的发生危象的诱因 3.具有下列系统性红斑狼疮恶化的临床表现: ①急进性狼疮性肾炎②严重的中枢神经系统损害③严重的溶血性贫血④血小板减少性紫癜⑤粒细胞缺乏症⑥严重心脏损害⑦严重狼疮性肺炎⑧严重狼疮性肝炎⑨严重的血管炎等凡具有1、2项和3项中的任何一条,即可考虑为狼疮危象  治疗 轻型SLE的药物治疗:•患者虽有疾病活动,但症状轻微,仅表现光过敏、皮疹、关节炎或轻度浆膜炎,而无明显内脏损害药物治疗包括:①非甾类类抗炎药(NSAIDs)  可用于控制关节炎应注意消化道溃疡、出血、肾、肝功能等方面的副作用②抗疟药  可控制皮疹和减轻光敏感,常用羟氯喹200mg,每日1~2次主要不良反应是眼底病变,用药超过6个月者,应每半年检查眼底有心脏病史者,特别是心动过缓或有传导阻滞者禁用抗疟药③可短期局部应用激素治疗皮疹,但脸部应尽量避免使用强效激素类外用药,一旦使用,不应超过一周④小剂量激素(泼尼松≤10mg/d)可减轻症状⑤权衡利弊,必要时可用硫唑嘌呤、甲氨蝶呤或环磷酰胺等免疫抑制剂应注意轻型SLE可因过敏、感染、妊娠生育、环境变化等因素而加重,甚至进入狼疮危象。

      中度活动型狼疮的治疗•个体化糖皮质激素治疗是必要的,通常泼尼松剂量0.5-1mg/kg/d需要联合其他免疫抑制剂,如:①甲氨蝶呤②硫唑嘌呤 重型SLE的治疗:•治疗主要分两个阶段,即诱导缓解和巩固治疗诱导缓解目的在于迅速控制病情,阻止或逆转内脏损害,力求疾病完全缓解(包括血清学指标、症状和受损器官的功能恢复),但应注意过分免疫抑制诱发的并发症,尤其是感染、性腺抑制等目前,多数患者的诱导缓解期需要超过半年至1年才能达到缓解,不可急于求成  Drugs•糖皮质激素(glucocorticoids )•非甾类类抗炎药(NSAIDs) •羟氯喹  (Hydroxychloroquine )•环磷酰胺(Cyclophosphamide,  CTX) •硫唑嘌呤(Azathioprine)•甲氨蝶呤(Methotrexate,MTX) •环孢素(Cyclosporine)•霉酚酸酯( Mycophenolate mofetil)  狼疮危象的治疗   治疗目的在于挽救生命、保护受累脏器、防止后遗症通常需要大剂量甲基泼尼松龙冲击治疗,针对受累脏器的对症治疗和支持治疗,以帮助患者度过危象后继的治疗可按照重型SLE的原则,继续诱导缓解和维持巩固治疗。

         ①急进性肾小球肾炎:•表现为急性进行性少尿,浮肿,蛋白尿/血尿,低蛋白血症,贫血,肾功能进行性下降,血压增高,高血钾,代谢性酸中毒等B超肾脏体积常增大,肾脏病理往往呈新月体肾炎,多符合WHO的LN的Ⅳ型 ①急进性肾小球肾炎--治疗•纠正水电解质酸碱平衡紊乱、低蛋白血症,•防治感染,•纠正高血压,心衰等合并症,为保护重要脏器,必要时需要透析支持治疗•为判断肾损害的急慢性指标,明确肾损病理类型,制定治疗方案和判断预后,应抓住时机肾穿•对明显活动、非纤维化/硬化等不可逆病变为主的患者,应积极使用激素(泼尼松≥2mg/kg/d),或使用大剂量MP冲击疗法,同时用环磷酰胺0.4~0.8g,每2周静脉冲击治疗  ②神经精神狼疮•严重的抽搐,急性意识障碍,昏迷,脑卒中,横贯性脊髓炎,精神性发作,脱髓鞘综合征等 ②神经精神狼疮:治疗•必须除外化脓性脑膜炎、结核性脑膜炎、隐球菌性脑膜炎、病毒性脑膜脑炎等中枢神经系统感染•弥漫性神经精神狼疮在控制SLE的基础药物上强调对症治疗,包括抗精神病药物(与精神科医生配合),癫痫大发作或癫痫持续状态时需积极抗癫痫治疗,注意加强护理•ACL相关神经精神狼疮,应加用抗凝、抗血小板聚集药物。

      •有全身血管炎表现的明显活动证据,应用大剂量MP冲击治疗•中枢狼疮包括横贯性脊髓炎在内,在除外中枢神经系统感染的情况下,可试用地塞米松10mg,或地塞米松10mg加MTX10mg鞘内注射/每周,共2~3次  ③重症血小板减少性紫癜:•血小板<2万/mm3,有自发出血倾向•常规激素治疗无效(1mg/kg/d),应加大激素用量用至2mg/kg/d以上•还可静脉滴注长春新碱(VCR)1~2mg/每周×3~6次•静脉输注大剂量人静脉用免疫球蛋白(IVIG)对重症血小板减少性紫癜有效,可按0.4g/kg/d,静脉滴注,连续3~5天为1个疗程IVIG一方面对SLE本身具有免疫治疗作用,另一方面具有非特异性的抗感染作用,可以对大剂量甲基泼尼松龙和环磷酰胺的联合冲击治疗所致的免疫力挫伤起到一定的保护作用,能够明显提高各种狼疮危象治疗的成功率•无骨髓增生低下的重症血小板减少性紫癜还可试用其他免疫抑制剂,如环磷酰胺,环孢素等•其他药物包括达那唑、三苯氧胺、维生素C等•内科保守治疗无效,可考虑脾切除  ④弥漫性出血性肺泡炎和急性重症肺间质病变:• 呼吸困难、咯血、低氧血症等•部分弥漫性出血性肺泡炎的患者起病可无咯血,支气管镜有助于明确诊断。

      本病极易合并感染,常同时有大量蛋白尿,预后很差迄今无治疗良策•对SLE肺脏累及应提高警惕,结合SLE病情系统评估,影像学、血气分析和纤维支气管镜等手段,以求早期发现、及时诊断 ④弥漫性出血性肺泡炎和急性重症肺间质病变:•治疗包括氧疗、必要时机械通气,控制感染和支持治疗•可试用大剂量MP冲击治疗,IVIG和血浆置换    ⑤严重的肠系膜血管炎:•常需2mg/kg/d以上的激素剂量方能控制病情应注意水电解质酸碱平衡,加强肠外营养支持,防治合并感染,避免不必要的手术探查一旦并发肠坏死、穿孔、中毒性肠麻痹,应及时手术治疗  特殊治疗:•血浆置换等治疗不宜列入常规治疗,应视患者具体情况选择应用•丙种球蛋白 0.4g/kg/天, 5-7天 免疫抑制剂免疫抑制剂                                          免疫抑制剂•糖皮质激素糖皮质激素(glucocorticoids ) •羟氯喹羟氯喹  (Hydroxychloroquine )•环磷酰胺(环磷酰胺(Cyclophosphamide,  CTX)) •硫唑嘌呤(硫唑嘌呤(Azathioprine))•甲氨蝶呤(甲氨蝶呤(Methotrexate,,MTX)) •环孢素(环孢素(Cyclosporine))•霉酚酸酯霉酚酸酯( Mycophenolate mofetil) •CD20单抗单抗 抗疟药-羟氯喹•药代动力学   据报道硫酸羟氯喹片经口服后在眼、肾、肝、肺等器官广泛分布,也可通过胎盘,约2~4.5小时血药浓度达峰值,红细胞中的浓度高于血浆浓度2~5倍。

      部分在肝脏代谢为具有活性的脱乙基代谢物血浆消除半衰期约32天主要经肾排泄,排泄缓慢,其中23—25%为原形药物,也能随乳汁排泄•起效相对缓慢,用药后3-6个月始能见效(氯喹起效快,一般用药1-2月即可见效)  抗疟药-羟氯喹•禁忌症1.已知对4-氨基喹啉类化合物过敏的患者2.先前存在眼睛黄斑病变的患者,治疗后出现视网膜或视野改变者3.肝病患者4.孕妇5.新生儿,6岁以下儿童(200mg片剂不适合于体重小于35kg的儿童) 抗疟药-羟氯喹•用法和剂量   SLE皮损、关节炎、口腔溃疡、乏力有效氯喹:250mg/d羟氯喹:200mg Bid纷乐(国产)0.1/片赛能 (进口)0.2/片 抗疟药-羟氯喹•不良反应1.眼毒性2.皮疹:羟氯喹可促进银屑病的发作发热和白细胞增多可能与羟氯喹有关3.胃肠不适4.神经肌肉病变5.心血管:心脏传导异常及双侧心室肥厚时,应怀疑到药物的慢性毒性作用6.血液系统:骨髓抑制罕见:WBC,HB,PLT 环磷酰胺•药理毒理:本品在体外无活性,进入体内被肝脏或肿瘤内存在的过量的磷酰胺酶或磷酸酶水解,变为活化作用型的磷酰胺氮芥而起作用其作用机制与氮芥相似,与DNA发生交叉联结,抑制DNA的合成,也可干扰RNA的功能,属细胞周期非特异性药物。

      本品抗瘤谱广,对多种肿瘤有抑制作用  环磷酰胺•药代动力学 :环磷酰胺口服易吸收,迅速分布全身,约1小时后达血浆峰浓度,在肝脏转化释出磷酰胺氮芥,其代谢产物约50%与蛋白结合静注后血浆半衰期4~6小时,48小时内经肾脏排出50%~70%,其中68%为代谢产物,32%为原形  环磷酰胺•成人常用量:单药静脉注射按体表面积每次500~1000mg/m2,加生理盐水,静脉滴入,每周1次,连用2次,休息1~2周重复联合用药500~600mg/m2儿童常用量:静脉注射每次10~15mg/kg,加生理盐水20ml稀释后缓慢注射,每周1次,连用2次,休息1~2周重复也可肌内注射  环磷酰胺•禁忌禁忌 :1.对本品过敏者禁用2.妊娠及哺乳期妇女禁用•慎用慎用:1.凡有骨髓抑制、感染、肝肾功能损害2.痛风病史3.肿瘤细胞浸润致骨髓4.泌尿系结石5.有多程化疗或放疗者 环磷酰胺•注意事项 :本品的代谢产物对尿路有刺激性,应用时应鼓励患者多饮水,大剂量应用时应水化、利尿,同时给予尿路保护剂美司钠近年研究显示,提高药物剂量强度,能明显增加疗效,当大剂量用药时,除应密切观察骨髓功能外,尤其要注意非血液学毒性如心肌炎、中毒性肝炎及肺纤维化等。

      当肝肾功能损害、骨髓转移或既往曾接受多程化放疗时,环磷酰胺的剂量应减少至治疗量的1/2~1/3由于本品需在肝内活化,因此腔内给药无直接作用环磷酰胺水溶液仅能稳定2~3小时,最好现配现用  环磷酰胺•药物相互作用:环磷酰胺可使血清中假胆碱酯酶减少,使血清尿酸水平增高,因此,与抗痛风药如别嘌呤醇、秋水仙碱、丙磺舒等同用时,应调整抗痛风药物的剂量此外也加强了琥珀胆碱的神经肌肉阻滞作用,可使呼吸暂停延长环磷酰胺可抑制胆碱酯酶活性,因而延长可卡因的作用并增加毒性大剂量巴比妥类、皮质激素类药物可影响环磷酰胺的代谢,同时应用可增加环磷酰胺的急性毒性  环磷酰胺•是治疗SLE最常用的免疫抑制剂,一般用于有脏器或组织损害者,如狼疮肾炎、精神神经狼疮、血管炎、血小板减少和间质性肺炎•激素依赖或效果不佳•IV/V型狼疮肾炎•消化系统血管炎、对于大剂量激素或切脾反应不好的顽固性血栓性血小板、多发性单神经炎、视神经病变、多肌炎、弥漫性牙龈出血 环磷酰胺•每月冲击一次,剂量0.5-1.0g/m2,即800-1000mg/次,6个月后用药间隔延长,并再用12-24月(6周-8周-12周)•隔日200mg•每周400mg•600-800mg每2周•总量3克左右即应起效•6-8g病情可明显缓解•﹥12g,蛋白尿不好转,应加用其它免疫抑制剂 环磷酰胺•是主要作用于S期的细胞周期特异性烷化剂,通过影响DNA合成发挥细胞毒作用。

      其对体液免疫的抑制作用较强,能抑制B细胞增殖和抗体生成,且抑制作用较持久,是治疗重症SLE的有效的药物之一,尤其是在狼疮性肾炎和血管炎的患者中,环磷酰胺与激素联合治疗能有效地诱导疾病缓解,阻止和逆转病变的发展,改善远期预后•目前普遍采用的标准环磷酰胺冲击疗法是:0.5~1.0g/m2体表面积,加入生理盐水250ml中静脉滴注,每3~4周1次,个别难治、危重患者可缩短冲击间期多数患者6~12个月后病情缓解,而在巩固治疗阶段,常需要继续环磷酰胺冲击治疗,逐渐延长用药间歇期,至约三个月一次维持数年过去认为环磷酰胺累积剂量不应超过9~12g以上,新近的研究提示,环磷酰胺累积剂量并不受此限制但是,由于各人对环磷酰胺的敏感性存在个体差异,年龄、病情、病程和体质使其对药物的耐受性有所区别,所以治疗时应根据患者的具体情况,掌握好剂量、冲击间隔期和疗程,既要达到疗效,又要避免不良反应 环磷酰胺•白细胞计数对指导环磷酰胺治疗有重要意义,治疗中应注意避免导致白细胞过低,一般要求白细胞低谷不小于3.0×109/L环磷酰胺冲击治疗对白细胞影响有一定规律,一次大剂量环磷酰胺进入体内,第3天左右白细胞开始下降,7~14天至低谷,之后白细胞逐渐上升,至21天左右恢复正常。

      对于间隔期少于3周者,应更密切注意血象监测大剂量冲击前需查血常规•除白细胞减少和诱发感染外,环磷酰胺冲击治疗的副作用包括:性腺抑制(尤其是女性的卵巢功能衰竭)、胃肠道反应、脱发、肝功能损害,少见远期致癌作用(主要是淋巴瘤等血液系统肿瘤),出血性膀胱炎、膀胱纤维化和长期口服而导致的膀胱癌  硫唑嘌呤•硫唑嘌呤为嘌呤类似物,抑制腺嘌呤和鸟嘌呤的合成,进而抑制DNA合成 硫唑嘌呤药理作用:硫唑嘌呤为嘌呤类似物,抑制腺嘌呤和鸟嘌呤的合成,进而抑制DNA合成动物实验证实,本药可使胸腺、脾内DNA、RNA减少,影响DNA、RNA,以及蛋白质的合成,主要抑制T-淋巴细胞而影响免疫,所以可抑制迟发过敏反应,器官移植的排斥反应本药的疗效需于治疗数周或数月后才出现在上消化道内吸收较佳血浆中的硫唑嘌呤及6-硫基嘌呤水平与本药的疗效及毒性无相互关系  硫唑嘌呤•硫唑嘌呤(azathioprine):疗效不及环磷酰胺冲击疗法,尤其在控制肾脏和神经系统病变效果较差,而对浆膜炎、血液系统、皮疹等较好•用法用法:1~2.5mg/(kg•d),常用剂量50~100mg/d副作用包括:骨髓抑制、胃肠道反应、肝功能损害等少数对硫唑嘌呤极敏感者用药短期就可出现严重脱发和造血危象,引起严重粒细胞和血小板缺乏症,轻者停药后血象多在2~3周内恢复正常,重者则需按粒细胞缺乏或急性再障处理,以后不宜再用。

        硫唑嘌呤•禁忌症1.  对本药及6-疏基嘌呤过敏者禁用2.       肝肾功能不全 3.       可致畸胎,孕妇忌用•别嘌呤醇可抑制巯基嘌呤(后者是硫唑嘌呤的活性代谢物)代谢成无活性产物,结果使巯基嘌呤的毒性增加,当二者必须同时服用时,硫唑嘌呤的剂量应该大大地减低•50mg/片依木兰 硫唑嘌呤•1、急慢性白血病,对慢性粒细胞型白血病近期疗效较好,作用快,但缓解期短;2、后天性溶血性贫血,特发性血小板减少性紫癜,系统性红斑狼疮;3、慢性类风湿性关节炎、慢性活动性肝炎(与自体免疫有关的肝炎)、原发性胆汁性肝硬变;4、甲状腺机能亢进,重症肌无力;5、其他:慢性非特异性溃疡性结肠炎、节段性肠炎、多发性神经根炎、狼疮性肾炎,增殖性肾炎,Wegener氏肉芽肿等 硫唑嘌呤•1、 CTD :1~2.5mg/(kg•d),常用剂量50~100mg/d•2、异体移植,每日2mg~5mg/kg,一日1次或分次口服; •3、白血病,每日1.5mg~3mg/kg,一日1次或分次口服  甲氨蝶呤 •为二氢叶酸还原酶拮抗剂,通过抑制核酸的合成发挥细胞毒作用疗效不及环磷酰胺冲击疗法,但长期用药耐受性较佳剂量10~15mg,每周1次,或依据病情适当加大剂量。

      主要用于关节炎、肌炎、浆膜炎和皮肤损害为主的SLE其副作用有胃肠道反应、口腔黏膜糜烂、肝功能损害、骨髓抑制,偶见甲氨蝶呤导致的肺炎和肺纤维化  甲氨蝶呤•【【禁忌症禁忌症】】1.已知对本品高度过敏的患者禁用2.孕妇3.极度衰竭恶液质4.肝肾功能不全5.骨髓抑制:周围血象如白细胞低于3500/mm3或血小板低于80000/mm3时不宜用 6.心肺功能不全•【注意事项】 1 本品的致突变性,致畸性和致癌性较烷化剂为轻,但长期服用后,有潜在的导致继发性肿瘤的危险;2 对生殖功能的影响,虽也较烷化剂类抗癌药为小,但亦可导致闭经和精子减少或缺乏,尤其是在长期应用较大剂量后,但一般多不严重,有时呈不可逆性; 甲氨蝶呤•【不良反应】   1胃肠道反应包括口腔炎口唇溃疡咽喉炎恶心呕吐腹痛腹泻消化道出血食欲减退常见偶见伪膜性或出血性肠炎等;2肝功能损害包括黄疸丙氨酸氨基转移酶碱性磷酸酶γ-谷氨酰转肽酶等增高长期口服可导致肝细胞坏死脂肪肝纤维化甚至肝硬变;3大剂量应用时由于本品和其代谢产物沉积在肾小管而致高尿酸血症肾病此时可出现血尿蛋白尿尿少氮质血症甚或尿毒症;4长期用药可引起咳嗽气短肺炎或肺纤维化;5骨髓抑制:主要为白细胞和血小板减少长期口服小剂量可导致明显骨髓抑制贫血和血小板下降而伴皮肤或内脏出血;6脱发皮肤发红瘙痒或皮疹;7细胞低下时可并发感染 甲氨蝶呤•用量小于30mg/m2时,口服吸收良好,1小时~5小时血药浓度达最高峰。

      部分经肝细胞代谢转化为谷氨酸盐,另有部分通过胃肠道细菌代谢主要经肾(约40~90%)排泄,大多以原形药排出体外;小于10%的药物通过胆汁排泄,T1/2α为1小时;T1/2β为二室型:初期为2~3小时;终末期为8~10小时少量甲氨喋呤及其代谢产物可以结合型形式贮存于肾脏和肝脏等组织中长达数月,在有胸腔或腹腔积液情况下,本品的清除速度明显减缓清除率个体差别极大,老年患者更甚  甲氨蝶呤•【禁忌症】 –甲氨蝶呤一般禁用于肾功能不良的病人治疗牛皮癣时,甲氨蝶呤禁忌用于孕妇及营养不良的病人、严重肝、肾功能不良者或原有血液病,如骨髓增生低下、白细胞减少症、血小板减少症或贫血的病人甲氨蝶呤忌用于明显或实验室证据表明免疫不良综合症者妇女口服甲氨蝶呤者禁忌哺乳 环孢素•环孢素(cyclosporine):可特异性抑制T淋巴细胞产生IL-2,发挥选择性的细胞免疫抑制作用,是一种非细胞毒免疫抑制剂对狼疮性肾炎(特别是V型LN)有效,环孢素剂量3~5mg/(kg•d),分两次口服•用药期间注意肝、肾功能及高血压、高尿酸血症、高血钾等,有条件者应测血药浓度,调整剂量,血肌酐较用药前升高30%,需要减药或停药环孢素对LN的总体疗效不如环磷酰胺冲击疗法,且价格昂贵、毒副作用较大、停药后病情容易反跳等。

        环孢素•禁忌症禁忌症: 1.过敏2.病毒感染:水痘、带状疱疹3.恶性肿瘤4.免疫缺陷5.严重心肺疾患6.嗜睡者7.未控制的高血压患者8.严重肾功能不全成人最高不超过200微摩尔/升,儿童最高不超过140微摩尔/升)9.吸毒者10.孕妇和哺乳期妇女11.1岁以下儿童 环孢素慎用:慎用:1.肝功能不全2.高钾血症3.高尿酸血症4.感染性疾病5.>65岁患者 环孢素用法:用法:1.RA:3mg/kg 分2次口服--5mg/kg2.LN:  4-5mg/kg 分2-3次口服3.肾病综合症:5mg/kg  若肾功能不全 2.5mg/kg    3个月无效停药全血:250-800ng/ml血浆:50-300ng/ml新赛斯平,新山地明 霉酚酸酯•霉酚酸酯:为次黄嘌呤单核苷酸脱氢酶抑制剂,可抑制嘌呤从头合成途径,从而抑制淋巴细胞活化治疗狼疮性肾炎有效,能够有效的控制IV型LN活动剂量为10~30mg/(kg•d),分2次口服  霉酚酸酯•口服吸收迅速,吸收率平均为口服吸收迅速,吸收率平均为94.1%单剂口服后约单剂口服后约40min~~1h血浆药物浓度达高峰,血浆蛋白结合率高达血浆药物浓度达高峰,血浆蛋白结合率高达98%,只有少量游离的%,只有少量游离的MPA发挥生物学活性。

      发挥生物学活性•霉酚酸酯霉酚酸酯 ((Mycophenolate mofetil, MMF)是霉酚酸)是霉酚酸((mycophenolic acid, MPA)的酯类衍生物,现通称)的酯类衍生物,现通称吗吗替麦考酚酯替麦考酚酯(商品名:(商品名:骁悉骁悉 CellCept),具有独特的免),具有独特的免疫抑制作用和较高的安全性疫抑制作用和较高的安全性MPA经肝代谢,绝大部分代经肝代谢,绝大部分代谢产物随胆汁排入小肠,在肠道细菌作用下重新转化为谢产物随胆汁排入小肠,在肠道细菌作用下重新转化为MPA,经门脉入血形成肝肠循环,约,经门脉入血形成肝肠循环,约10~~12h出现第二次出现第二次血药浓度高峰,血药浓度高峰,t1/2为为16~~17h代谢产物主要经肾排出,代谢产物主要经肾排出,严重肾功能不全者应减少严重肾功能不全者应减少MMF用量用量  霉酚酸酯•临床应用  1. 器官移植 主要用于肾脏、肝脏及心移植,能显著减少急性排斥反应的发生   2. 自身免疫性疾病 用于银屑病和类风湿性关节炎疗效较好,对系统性红斑狼疮血管炎、LN、重症IgA肾病也有一定效果  霉酚酸酯•不良反应    MMF与azathioprine和cyclosporin相比,最大的优点是无明显的肝肾毒性。

      常见不良反应有胃肠道反应,表现为恶心、呕吐、腹泻、腹痛等,通过调整剂量即可减轻;贫血和白细胞减少,多为轻度,通常发生在30~120d,大部分病例在停药一周后可得到缓解;机会感染轻度增加;可能诱发肿瘤动物试验证明MMF有致畸作用,而且MMF可分泌到乳汁中,因而育龄妇女应用时要注意避孕  霉酚酸酯禁忌症   对 MMF 或 MPA 发生过敏反应的病人不能使用本药  霉酚酸酯•注意事项1.  服用本药的病人在第一个月每周 1 次进行全血细胞计数,第二和第三个月每月 2 次,余下的一年中每月 1 次,如果发生中性粒细胞减少 ( 中性粒细胞绝对计数小于 1.3 x 10 3 /uL) 时,应停止或减量使用本药,并对这些病人密切观察 2.  严重慢性肾功能损害 ( 肾小球滤过率小于 25 mL/ 分 /1.73 m 2 ) 的病人服用单剂量后,血浆 MPA 和 MPAG 的曲线下面积比轻度肾功能损害的病人及健康人高应避免使用超过 1 g/ 次,每日 2 次的剂量,并且应对这些病人密切观察 3.  肾移植后肾功能恢复的病人,平均 0-12 小时 MPA 曲线下面积与正常恢复病人相仿但 MPAG 的 0-12 小时曲线下面积前者比后者高 2-3 倍。

      对这些肾功能延迟恢复的病人无须作剂量调整,但应密切观察 4.  接受免疫抑制疗法的病人常使用联合用药方式本药作为联合应用免疫抑制药物时,有增加淋巴瘤和其它恶性肿瘤 ( 特别是皮肤瘤 ) 发生的危险这一危险性与免疫抑制的强度和持续时间关,而不是与某一特定药物有关免疫系统的过度抑制也可能导致对感染的易感性增加临床试验中,本药已与以下药物联合应用 :抗淋巴细胞球抗体、环孢素和皮质激素类药物,以预防排斥反应和治疗难治性排斥反应  霉酚酸酯药物过量  在人类尚无 MMF 剂量过大的报道血透不能清除 MPA  MPA 可通过药物排出增加 ( 如给予消胆胺 ) 而得到清除  霉酚酸酯•药物相互作用1.  不能与硫唑嘌呤同时使用,对这两种药物的同时使用尚未进行试验 2.  已注意到消胆胺能显著减少 MPA 线曲下面积 3.  本药不应与能干扰肠肝再循环的药物同时使用,因这些药物可能会降低本药的药效 4.  和阿昔洛韦同时服用时, MPAG 和阿昔洛韦 的血浆浓度较两种药物单独服用时为高当肾功损害时, MPAG 和阿昔洛韦的血浆浓度都升高这两种药物竞争性地通过肾小管排出,可能会进一步增加两种药物的浓度 5.  制酸药物和氢氧化镁以及氢氧化铝 :同时服用制酸剂时, MMF 的吸收减少。

       6.  消胆胺 :健康人先期服用消胆胺 4 g/ 次,每日 3 次, 4 日后给予单剂量 MMF 1.5 g , MPA 的曲线下面积减少 40%  7.  环孢素 A 的药物动力学不受 MMF 的影响 8.  更昔洛韦 :未观察到 MMF 和静脉注射更昔洛韦之间有药物动力学的交叉作用 9.  口服避孕药 :目前尚未发现 MMF 和口服避孕药 1 mg 炔诺酮 /35 ug 炔雌醇之间有相互影响但这只是单次剂量研究所得出的结论,并不能排除长期服用本药后改变口服避孕药的药物动力学的可能性这可能导致口服避孕药的药效降低 10.  磺胺甲基异唑 :对 MPA 的生物利用度无影响 11.  其它药物 :给猴子同时服用丙磺舒和 MMF ,可使血浆 MPAG 曲线下面积增加 3 倍为此,其它经肾小管排出的药物可与 MPAG 竞争,从而使血浆 MPAG 或这些药物的浓度升高  雷公藤•可用于类风湿性关节炎、原发性肾小球肾病、肾病综合征、紫瘢性及狼疮性肾炎、红斑狼疮、亚急性及慢性重症肝炎、慢性活动性肝炎;亦可用于过敏性皮肤脉管炎、皮炎和湿疹,以及银屑病性关节炎、麻风反应、白噻氏病、复发性口疮、强直性脊柱炎等。

      雷公藤•【药理作用】本品具有较强的抗炎及免疫抑制作用在抗炎作用方面,它能拮抗和抑制炎症介质的释放、实验性炎症及关节炎的反应程度在抑制免疫作用方面,它能抑制体液免疫和细胞免疫反应  雷公藤•【不良反应】主要为胃肠反应,一般可耐受偶可见血小板减少,停药后可恢复可致月经紊乱及精子活力降低禁忌症】孕妇和哺乳期妇女产品规格】10mg用法用量】 口服10-20mg tid,病情控制后减量 来氟米特•【药理毒理】本品为一个具有抗增殖活性的异噁唑类免疫抑制剂,其作用机理主要是抑制二氢乳清酸脱氢酶的活性,从而影响活化淋巴细胞的嘧啶合成体内外试验表明本品具有抗炎作用来氟米特的体内活性主要通过其活性代谢产物A771726(M1)而产生 来氟米特•【药代动力学】本品口服吸收迅速,在胃肠粘膜与肝中迅速转变为活性代谢产物A771726(M1),口服后6~12小时内A771726的血药浓度达峰值,口服生物利用度(F)约80%,吸收不受高脂肪饮食影响单次口服50mg或100mg后24小时,血浆A771726浓度分别为4μg /ml或8.5μg /mlA771726主要分布于肝、肾和皮肤组织,而脑组织分布较少;A771726血浆浓度较低,血浆蛋白结合率大于99%,稳态分布容积为0.13L/kg。

      A771726在体内进一步代谢,并从肾脏与胆汁排泄,其半衰期约10天 来氟米特•【药物过量】据文献报道,如果剂量过大或出现毒性时,可给予考来烯胺或活性炭加以消除方法:(1)口服考来烯胺(8g,3次/24小时),24小时内M1血浆浓度降低约40%,48小时内降低大约49%~65%连续服用11天,M1血浆浓度可降至0.02μg /m1以下2)通过胃管或口服给予活性炭(混悬液),每6小时50克,24小时内M1血浆浓度降低37%,48小时降低48%如果临床上需要,这些措施可以重复使用 来氟米特•【药物相互作用】据文献资料报道:(1)考来烯胺和活性炭 13例患者和96例志愿者给予考来烯胺或活性炭,血浆中Ml浓度很快减少2)肝毒性药物 来氟米特和其它肝毒性药物合用可能增加不良反应,同时也应考虑到虽然中断来氟米特治疗,但没有采取药物消除措施就接着服用这些药物,同样有可能增加不良反应在小样本(30例)来氟米特和MTX联合用药的研究中,有5例肝脏酶出现2~3倍升高其中2例继续服用,3例中断来氟米特治疗,酶的升高都得到恢复另外5例升高大于3倍,其中2例继续服用,3例中断来氟米特治疗,酶的升高也都得到恢复3)非甾体抗炎药物 在体外一系列临床研究中,Ml可使血浆游离双氯芬酸和布洛芬的浓度升高13%~50%,此临床意义还不清楚。

      但在临床试验中曾观察了许多和非甾体药物同时应用的病例,没有发现有特殊影响4)甲苯磺丁脲 在一系列临床研究中发现,Ml可使血浆游离甲苯磺丁脲浓度升高13%~50%,此临床意义还不清楚5)利福平 单剂量来氟米特和多剂量利福平联合使用,M1峰浓度较单独使用来氟米特升高(约40%),由于随着利福平的使用,Ml浓度可能继续升高,因此当两药合用时,应慎重 来氟米特【注意事项】1.临床试验发现来氟米特可引起一过性的ALT升高和白细胞下降,服药初始阶段应定期检查ALT和白细胞检查间隔视病人情况而定2.有肝脏损害和明确的乙肝或丙肝血清学指标阳性的患者慎用用药前及用药后每月检查ALT,检测时间间隔视病人具体情况而定如果用药期间出现ALT升高,调整剂量或中断治疗的原则:①如果ALT升高在正常值的2倍(<80U/L)以内,继续观察②如果ALT升高在正常值的2~3倍之间(80~120U/L),减半量服用,继续观察,若ALT继续升高或仍然维持80~120U/L之间,应中断治疗③如果ALT升高超过正常值的3倍(>120U/L),应停药观察停药后若ALT恢复正常可继续用药,同时加强护肝治疗及随访,多数病人ALT不会再次升高。

      来氟米特•3.免疫缺陷、未控制的感染、活动性胃肠道疾病、肾功能不全、骨髓发育不良(bone marrowdysplasia)的患者慎用4.如果服药期间出现白细胞下降,调整剂量或中断治疗的原则如下1)若白细胞不低于3.0×109/L,继续服药观察2)若白细胞在2.0×109/L~3.0×109/L之间,减半量服药观察继续用药期间,多数病人可以恢复正常若复查白细胞仍低于3.0×109/L,中断服药3)若白细胞低于2.0×109/L,中断服药建议粒细胞计数不低于1.5×109/L5.准备生育的男性应考虑中断服药,同时服用考来烯胺(消胆胺)6.在本品治疗期间接种免疫活疫苗的效果和安全性没有临床资料,因此服药期间不应使用免疫活疫苗7.据国外报道,该药在国外上市后罕见间质性肺炎的发生,有肺部疾患者,请慎用或遵医嘱 来氟米特禁忌症:1. 对来氟米特及其代谢产物过敏者;       2. 孕妇哺乳期妇女【禁忌】对本品及其代谢产物过敏者及严重肝脏损害患者禁用   来氟米特•【不良反应】用于类风湿关节炎的治疗:主要有腹泻、瘙痒、可逆性肝脏酶(ALT和AST)升高、脱发、皮疹等在国外临床试验中,来氟米特治疗1339例类风湿关节炎病人中,发生率≥3%的不良事件包括:乏力、腹痛、背痛、高血压、厌食、腹泻、消化不良、胃肠炎、肝脏酶升高、恶心、口腔溃疡、呕吐、体重减轻、关节功能障碍、腱鞘炎、头晕、头痛、支气管炎、咳嗽、呼吸道感染、咽炎、脱发、瘙痒、皮疹、泌尿系统感染等。

      以上不良事件均在安慰剂对照或阳性对照柳氮磺胺吡啶治疗组及MTX治疗组中发现,其中来氟米特治疗组以腹泻、肝脏酶升高、脱发、皮疹较为明显,在应用过程中应加以注意用于狼疮性肾炎的治疗:国内临床试验资料显示,来氟米特治疗108例活动增殖型狼疮肾炎病人中,前6个月(20-40mg/日),共有43人发生与试验药物可能有关/有关的不良反应,发生率为39.81%;治疗期间,发生率≥3%的不良事件包括:脱发、血压升高、带状疱疹、转氨酶升高、腹泻/稀便、白细胞下降、皮疹、月经不调、心悸、腹痛;发生率<3%的不良事件包括:恶心/呕吐、上呼吸道感染、血小板下降、乏力、胃烧灼感、厌食、发热、牙周疼痛、视觉异常、尿路感染、咽痛、巨细胞病毒感染、体重下降、多毛、肺部感染等治疗7-72个月(10-30mg/日),随访52例患者,共有12人发生与试验药物可能有关/有关的不良反应,发生率为23.08%,发生率≥3%的不良事件包括:白细胞下降、转氨酶升高、血小板减少;发生率<3%的不良事件包括:上呼吸道感染、脱发、带状疱疹、月经不调、尿路感染 来氟米特•【用法用量】(1)成人类风湿性关节炎:口服由于来氟米特半衰期较长,建议间隔24小时给药。
