RENAL评分系统在肾癌肾部分切除术中的应用 lR.E.N.A.L.评分系统是由KutikovKutikov等等【【1 1】】在2009年第一次提出的，该系统是一个术前肾肿瘤评分系统l该系统对预行保留肾单位手术的患者基于肾肿瘤影像学解剖特征进行术前肾肿瘤综合评估，客观评价肾脏肿瘤的手术难度以达到指导临床医师合理选择手术方式和治疗策略的目的R.E.N.A.L.评分计算方法l对每一变量进行量化评分后，每组得分按首字母缩写的顺序相加得到总分R.E.N.A.L.评分举例（左肾上极肿瘤）￿R.E.N.A.L.评分举例l肿瘤大小4.1cm=2分l外凸率<50%=2分l距肾盂距离<4mm=3分l腹侧/背侧=Xl肿瘤100%位于肾上极=1分lR.E.N.A.L.评分：2+2+3+X+1=8XR.E.N.A.L.评分系统稳定性l评分系统变量应具有稳定性和可重复性，同一肿瘤的评分不以评分者的改变而表现出显著差异lMontag￿【【2 2】】等分别采用组内相关分析和Kendall’s￿taub相关分析验证R.E.N.A.L.评分系统具备良好的稳定性R.E.N.A.L.评分系统与手术方式相关性lR.E.N.A.L.评分系统与手术方式具有显著相关性。

lCanter等【【3 3】】证实R.E.N.A.L.评分系统与手术方式具有显著相关性l随着肾肿瘤R.E.N.A.L.评分的增高（手术复杂程度的增加），选择开放保留肾单位手术的患者在比例逐渐增加，当R.E.N.A.L.评分>9分时，开放手术的比例明显多于腹腔镜手术R.E.N.A.L.评分系统与手术方式相关性l国内刘强等【4】研究表明：在临床上，患者的RENAL的评分越低，临床医生更倾向于肾部分切除术术式，反之倾向于肾根治性切除术这与国际上很多的报道结果一致【5-6】该研究也显示，与术式的选择存在直接关系的是年龄、R、N、L， R、N、L3项的单项的得分越高及患者的年龄越大，更倾向于采用RN术式，反之则倾向于PNR.E.N.A.L.评分与围手术期并发症关系l存在争议lCanter等￿【【3 3】】在证实R.E.N.A.L.评分系统与手术方式具有显著相关性的同时也认为该评分系统与围手术期并发症无显著相关性lHew等【【7 7】】认为R.E.N.A.L.评分≥9分是发生围手术期并发症的高危因素R.E.N.A.L.评分与术后并发症发生率之间的相关性lBruner等【【8 8】】报道称，R.E.N.A.L.评分每增加1分，术后尿漏的概率增加35%。
